Localization of the accessory pathway in ventricular preexcitation (WPW) by means of combined ECG and VCG recordings.

چکیده
The noninvasive localization of the accessory pa thway (AP) responsible for ventricular preexcitation is helpful in the initial evaluation of patients with Wolff-Parkinson-White (WPW) syndrome. Recently, two groups of authors independently published new classifications derived from the correlation of 12-lead ECG with electrophysiologic studies. ~'2 Although these classifications allow a more precise localization of the accessory pathway than the Rosenbaum criteria, 3 some cases remain difficult to classify. We used the Frank vectorcardiogram (VCG) to increase the accuracy and to simplify classification.

متن کاملElectrocardiographic Preexcitation and Risk of Cardiovascular Morbidity and Mortality
The Wolf–Parkinson–White (WPW) syndrome is a clinical entity characterized by the presence of ≥1 accessory pathways between the atria and the ventricles pre-disposing patients to arrhythmias. Anterograde conduction through the accessory pathway leads to preexcitation of the ventricles and a delta wave in the ECG. The prevalence of preexcitation in the general population has been estimated to be...
متن کاملChanging QRS Morphology: What is the mechanism?
ECG in sinus rhythm with ventricular preexcitation and changing QRS morphology was seen that was initially interpreted as the multiple accessory pathway from elsewhere. (Figure 1A). The following mechanisms are potentially involved in the electrogenesis of changing QRS morphology in WPW syndrome: 1) multiple accessory pathways1; 2) simultaneous occurrence of aberrant atrioventricular conduction...
متن کاملNoninvasive Localization of Cardiac Arrhythmia Sources from Multichannel Ecg Measurements
Results of noninvasive localization of arrhythmogenic tissue in the heart from multichannel ecg measurements are presented. Location of initial preexcitation was estimated by inverse computations using multiple dipole (MD) or jumping dipole (JD) model of equivalent heart generator. First, the influence of error factors frequently met in practice was analyzed on simulated ecg data. Mean localiza...
متن کاملDyssynchronous Ventricular Activation in Asymptomatic Wolff-Parkinson-White Syndrome: A Risk Factor for Development of Dilated Cardiomyopathy
A subset of children and adults with Wolff-Parkinson-White (WPW) syndrome develop dilated cardiomyopathy (DCM). Although DCM may occur in symptomatic WPW patients with sustained tachyarrhythmias, emerging evidence suggests that significant left ventricular dysfunction may arise in WPW in the absence of incessant tachyarrhythmias. An invariable electrophysiological feature in this non-tachyarrhy...
